We are seeking a Licensed Clinical Social Worker (LCSW) to support initiatives that address Indiana's opioid epidemic through the expansion of Mobile Crisis Teams in the Bloomington, Indiana area. 

LCSW would be 60% in field, 40% in office.

Typical Hours are M-F 8am-5pm

Primary duties of LCSW may include, but are not limited to:

  • Obtains an accurate member history, conduct an assessment of psychosocial factors, and assesses clinical information to develop care plans including a member support system. Working directly with substance abuse patients.
  • Utilizes skills and knowledge in psychiatric and chemical dependency assessment, negotiation, coordination, implementation, case planning, monitoring, and evaluating to promote quality member outcomes, to optimize member benefits, and to promote effective use of resources.
  • Establishes prioritized short and long-term goals in collaboration with the patient that meet the patients' needs and the referral source's requirements to positively impact a patient.
  • Must be flexible in utilizing a variety of methods in having a positive impact for a positive outcome.
  • Strong organizational skill set required. Ability to work in an independent setting with strong time management skill set.
  • Establishes working relationships with referral sources and community resources.
  • Identifies members with potential for high risk complications and coordinates the appropriate treatment in conjunction with the member and the health care team.
  • Communicates care objectives to appropriate individuals/departments/referral sources.
  • Provide case management and/or disease management services to members, as identified by a health plan's referral process.
  • May be required to perform service authorization duties using prescribed criteria and responsibilities as assigned and required.
  • Acts as an advocate for an individual's health care needs.
  • Reviews benefit systems and cost benefit analysis; evaluates the quality and necessity of applicable services; identifies members that would benefit from an alternative level of care; acquires data and evaluates necessary health services for cost containment; documents effectiveness of case management services
  • Coordinates resources and assist to effectively utilize resources
  • Gives guidance to support Health Care Needs
  • Other duties as requested or assigned.
